To report an incident where a dog or dogs caused injury to a person or another animal or to report an incident where a person was forced to take evasive action to avoid injury to themselves, someone else, or an animal.
Please include as much of the following information as possible:
Source of information (how did you find out about it)
City and province of incident
Date and time of incident (local time)
Victim information, including:
- age and sex of victim
- name of victim (if appropriate)
Dog information, including:
- Age, sex, breed, sexual status (spayed/neutered)
- name of dog
- name of dog owner (if appropriate)
- prior known incidents or complaints involving dog
- vaccination status
- training history
- source of dog (how did current owners come to be in possession of dog)
- parental information (if known)
- other history (if known)
- source(s) of above information
Relationship of victim to dog
Circumstances prior to incident, including:
- actions of victim
- behaviour of dog
- actions of other people/animals
- location of incident
- location of victim (especially in relation to dog)
- location of dog (chained, fenced, loose, in house)
Detailed description of incident, including how it was terminated
Actions taken after incident, including:
- emergency care and transportation of victim
- actions of police or animal control regarding dog
- current status of dog (dead or alive)
- if dead, how did it die (shooting, destruction at animal control facility, etc)
- current location of dog (dead or alive)
List of authorities notified, including:
- police
- ambulance personnel
- hospital
- animal control
- city health officials
Detailed report of injuries suffered, including expected permanence of those injuries
